The Virtual Primary Care Practice supports CU primary care clinics (15 clinics) with virtual same day appointments and provides a primarily virtual primary care experience for a group of empaneled patients.

Job Description

The APP evaluates, assesses, diagnoses, prescribes and educates patients regarding their particular illness or chronic disease.

The position provides high quality primary care to all patients, evaluates and treats acute and chronic problems for the patient population, completes histories, thorough assessments on all patients with accurate and complete documentation reflected in medical records and collaborates and communicates with other departments and colleagues, such as the pharmacy, to ensure optimal patient virtual care, including via telehealth appointments.

Depending on the needs and growth of the Virtual Primary Care Practice, a small portion of traditional, in-clinic primary care in the Denver Metro area may be necessary.

Examples of Work Performed Virtual Primary Care Provide full scope patient care in a virtual primary care setting. Participate as needed in chart review and clinical coding compliance activities.

Collaborate as appropriate with medical staff in the care of complex patients. Interact with other health professionals to create guidelines and coordinate patient care.

Collaborate with other team members in chronic disease management and patient care management. Support and collaborate with medical, nursing and office staff. Administrative/Professional Development Maintain eligibility for School of Medicine Faculty.

Key Responsibilities

• The Virtual Primary Care Practice supports CU primary care clinics (15 clinics) with virtual same day appointments and provides a primarily virtual primary care experience for a group of empaneled patients.
• The General Internal Medicine (GIM) Advanced Practice Provider (APP) in this role will be responsible for providing full spectrum virtual care including, but not limited to, chronic disease management, acute and “urgent” care, hospital follow up care, and wellness and preventative care for adult patients.
• Collaborate with other team members in chronic disease management and patient care management.
• Support and collaborate with medical, nursing and office staff.
• Accept other responsibilities assigned by the Medical Director.

• Proven effectiveness in working with individuals from diverse ethnic, cultural, and socio-economic backgrounds Willingness to be flexible with regards to work tasks and duties in an innovative clinic model.
• Ability to simultaneously perform multiple tasks and complete administrative duties correctly and on time.

Required Skills and Qualifications

• ­Keep abreast of the latest developments in field, both academically and through applied skill.
• Attend and present at conferences, meetings and trainings to maintain certification and enhance professional skills; participates in faculty development activities.
• Work Location: Remote – This role is eligible to work remotely, but the employee must be in the United States.
• These programs, when taken together, provide a wide breadth and depth of clinical experiences that allows our trainees to experience virtually all aspects of inpatient and ambulatory medical care.
• Qualifications: Minimum Qualifications Applicants must meet minimum qualifications at the time of hire.
• Faculty Rank is dependent upon years of experience, fellowships completed, and board certification.
• This is an open rank position and could be categorized as Instructor or Senior Instructor based on experience and qualifications as indicated below: Instructor: Master’s degree as a Nurse Practitioner or Physician Assistant Licensed as a Nurse Practitioner or Physician Assistant in the State of Colorado Medicare/Medicaid credentials Unrestricted DEA license Board certified or board eligible as a Nurse Practitioner or Physician Assistant Two (2) or more years of professional experience in a primary care setting Fellowships will count toward years of experience Senior Instructor: Medicare/Medicaid credentials.
